What financial institutions (Home office or branches) are doing business in Mahwah,NJ?

I pay a monthly installment for a "Fold 'n Go" mobility scooter to:
Department of Accounts
P.O. BOX 731
Mahwah, NJ 07430
and now want to pay the account off in full. I am loathe to do so, however, without the debt's owner bank/institution name, address and phone number-- and a 10 day payoff amount.
NOWHERE on the reverse of my monthly statement (light gray small print) is there an actual Institution name or phone number.
Any help anyone can shed is greatly appreciated. I will look up phone numbers of all institutions you are able to name for me (unless you know them <grin>)
