Where to get free downloads of how was our service slips to print?

hi ,i have just been ask to run front of house at a restaurant,budget is low for merchandise does anyone know any sites i can download templates or ready to print customer slips when we give them the bill,you know the ons that ask how was the food service ect they tick good bad poor and any comments,i really would love to fi nd these free as no funds for it at present but know they are great for getting feedback good or bad,i am not great on the pc so have not managed to type my own even after many attempts just looks tacky,any help with this will be great cheers.

Go to my most often used website: www.office.microsoft.com. It's basically the Microsoft Office Online page. Click on templates (in the left-hand column) and search for "customer service." A few variations pop up (4 pages worth.) All you need is the correct program (most are usually Word documents.) Click on the one you like and download it. You can make whatever changes you want to it and save to your computer!
