Is there a fee paid everytime I exchange funds from a mutual fund to another mutual fund within a 401k plan?

combination of first three answers. If there is a load on your funds then that is sort of a fee. If you have a brokerage account then you likely have transaction fees. If you are investing only in mutual funds with a firm other than a brokerage account then its possible (and most likely) to be no fees.

As long as it is a no-load fund, usually no.

Also check to make sure there isn't an early withdrawl penalty.

That depends on the 401(k) plan structure. If you have a self-directed brokerage account, then probably yes. If your plan is directly with a funds house, probably not. Check the plan details.
